MY AUS VISA — PRIVATE NO FURTHER STAY PREPARATION RECORD
Prepared 29 September 2026. Original aid, not an official request, eligibility decision or legal advice.
Keep this completed record and identity, medical, family and visa documents on your own device.
Do not send it to MyAusVisa. Use current Home Affairs instructions for the actual request.

1. MY ACTUAL VISA — one record for each affected person
[ ] Read my written grant and VEVO. Exact condition code: ____
Grant date: ____ Current visa/subclass: ____
Actual last authorised stay date from my own visa: ____
Date I checked VEVO: ____ Any ambiguity to clarify with qualified help: ____
[ ] Do not substitute visa travel validity for permitted stay.
[ ] The September policy announcement is not a record of my personal grant conditions.

2. CHANGE SINCE GRANT — explain truthfully, do not invent a qualifying reason
What happened after grant and on which date? ____
Why was it outside my control? ____
What makes the circumstances compassionate or compelling? ____
Which truthful documents support each point? ____
[ ] Check the current official examples and complete condition wording.
[ ] An example on a guide does not establish that my request qualifies.

3. REQUEST AND EVIDENCE
[ ] Confirm the current official channel for my condition and circumstances.
[ ] The online form names 8503, 8534 and 8535; Form 1447 also mentions 8540.
[ ] Check the current instructions rather than selecting an incorrect condition.
[ ] Prepare the passport biographical page and the evidence requested by Home Affairs.
[ ] Check the waiver-specific NAATI English translation requirement for non-English documents.
[ ] Each affected person submits an individual request and keeps their own receipt.
Private location of submitted copy, receipt and supporting records: ____

4. WHILE WAITING AND AFTER THE DECISION
[ ] A request does not waive a condition, extend my visa or keep me lawful after expiry.
[ ] Check every Department request and its individual deadline.
[ ] Do not treat a general processing indication as a guaranteed decision date.
[ ] If expiry is approaching or has passed, use official status help promptly.
Qualified adviser/official help question and date raised: ____
[ ] A granted waiver is not a grant of the new visa. Check that visa separately.
[ ] A waiver refusal is not reviewable by the ART; a new request needs substantially different circumstances.
Decision received/date and next official steps to check: ____
