COUNTRY-BY-COUNTRY HISTORY TO CHECK
Country or jurisdiction | Dates present since age 16 | Combined time if relevant | Official issuing authority | Requested by my visa instructions? | Certificate issue date
_______________________ | __________________________ | ________________________ | __________________________ | _________________________________ | _____________________
_______________________ | __________________________ | ________________________ | __________________________ | _________________________________ | _____________________
_______________________ | __________________________ | ________________________ | __________________________ | _________________________________ | _____________________

IF MY HISTORY INCLUDES SHIPS, YACHTS OR OIL RIGS
Vessel name | Flag country | Job | Employment dates | Good-conduct letter status
___________ | ____________ | ___ | ________________ | _________________________
[ ] Read the separate Home Affairs ship-worker instructions and my own request; do not assume residence countries alone cover vessel work or that a letter automatically replaces a certificate.

BEFORE I ORDER OR UPLOAD
[ ] Check the selected visa page and ImmiAccount request for who needs a certificate and when.
[ ] For a family application, check the exact subclass family definition and character-document instructions for migrating and non-migrating family members; do not assume only travelling people need consideration.
[ ] If an Australian check is required, use an AFP National Police Check with Commonwealth employment/purpose and Code 33; include all names used.
[ ] For another country, open Home Affairs Services outside Australia, select the country under Find a service outside Australia, then expand Police check and View more if provided; check each required country, not only current residence. If details do not load, retry rather than assuming no certificate is needed.
[ ] Check the certificate issue date, scope and any translation instructions for my visa.
[ ] For a non-English police certificate in a skilled-visa application, check the Home Affairs instruction to provide a certified copy of the translation alongside my exact subclass list and request.
Original file / English translation file / certification instruction checked: ____________________
Translator identification required by my instructions / included: ____________________
[ ] Check the period the certificate covers against my residence history and actual visa or request instructions.
Certificate country and person: ____________________
Period covered by certificate: ____________________
Period required by my official instructions: ____________________
Any gap to clarify through the official channel: ____________________
[ ] Declare requested charges and convictions truthfully even if a certificate appears clear.
[ ] If a certificate is unobtainable by the requested date, explain the issue through the official channel and follow Home Affairs instructions.
